Age Well East: Dementia Support Information & Advice for Wellbeing

Our service is tailored to meet the needs of people living with Dementia and their unpaid carers across North East Essex. Through a mixture of telephone, face to face and digital support we can assist with advice and information prior to diagnosis and through regular contact thereafter, identifying useful aids, support and information to help people to agewell within the community. We work closely with partners within the health and social care settings ensuring our service is both person centred and joined up to support positive outcomes.
