EPUT: Primary Care Mental Health Practitioners (North Essex)

The Primary Care Mental Health Practitioner (PCMHP) team at EPUT involves working with existing Primary Care teams in North East Essex GP practices. The practitioners triage and assess patients registered with GPs in the local area. The team refer patients to SUMMITs peer support workers. This referral pathway is for SUMMIT's PSW only to refer people they are working with back into a MHP when they recognise they may need additional clinical mental health support.
