Abbey Fields PCN: Social Prescribers

Abbey Field PCN accepts referrals, via Frontline, to their social prescribing service from ECC Adult Social Care teams. Patients must be registered at one of the following surgeries: Abbey Field Medical Centre, Elmstead Market Surgery, Hawthorn Surgery, The Tollgate Practice ,Ambrose Avenue Group Practice, Wivenhow Surgery and Rowhedge Surgery.
