St Helena: Care Choices Service
The Care Choices Service provide home visits to support patients with life limiting conditions, frailty and early stage dementia (patients must have capacity) to support them with planning their future and end-of-life care. Our team of trained volunteers visit patients in their homes to listen to what is important to them, discuss the choices available and complete the 'My Care Choices' and non-clinical parts of the ReSPECT form. The patient is then responsible to take this paperwork to their GP to complete and add to their notes.

Who can use this service?
Anyone with a life limiting condition, frailty or early stage dementia (patient must have capacity).
How are services delivered?
In the patients home with trained volunteers. Patients are responsible to take paperwork to their GP to sign off and add to their notes. Family are welcome to participate with the patients consent.
