The Multi Agency Neighbourhood Team (MAT) is a formalised framework for an integrated care network that blends distinct disciplines across the Health, Social Care, and community teams to operate under a unified model that supports the entire person and their environment. The MAT reviews the following resident cases identified under ‘PLACE’ as requiring additional support PLUS one of the following:

1. Resident on shared caseloads between different organisational partners

2. Resident considered a high intensity user for an organisation partner/s

3. Resident readmitting to an organisationpartner/s

4. No clear solution to keep resident independent and in control of their health wellbeing.

Each Neighbourhood includes these GP surgeries in their respective areas:

Neighbourhood Clacton Central includes Clacton Community Practices, East Lynne Medical Centre, North Clacton Medical Group, Old Road Surgery, Ranworth Surgery and St James Surgery.

Neighbourhood Colchester City includes Abbey Field Medical Centre, Creffield Medical Group, East Hill Surgery, Wimpole Road, Castle Garden and Shrub End.

Neighbourhood Colchester North includes Bluebell Surgery, Highwoods Surgery, Mill Road Surgery and Turner Road Surgery.

Neighbourhood Colchester South includes Ambrose Avenue Group Practice, Tiptree Medical Centre, Tollgate Health Centre, Mersea Island Medical Centre and Winstree Medical Practice.

Neighbourhood Greenstead and Colchester East includes Rowhedge & University of Essex, Wivenhoe Surgery, Parsons Health and Hawthorn.

Neighbourhood Harwich and Tendring North includes Fronks Road Family Surgery, Harewood Surgery, Lawford Surgery, Mayflower Medical Centre, The Ardleigh Surgery and The Riverside Health Centre.

Neighbourhood Tendring Rural includes Caradoc Surgery, Colne Medical Centre, Great Bentley Surgery, Thorpe Surgery and Walton Medical Centre.

Who can use this service?

See the criteria for a case review above. Only refer residents who are identified as
requiring additional support under ‘PLACE’, PLUS one of the criteria listed above.

How are services delivered?

Neighbourhood Teams work within each of the Neighbourhood Networks, providing wrap-around multi agency support for households with multiple and complex needs. The North East Essex Neighbourhood Model is about health, care and community teams working as one team locally to support people and their families. We focus on what matters most to them and use the ‘think PLACE' approach to look at their whole situation - so we can provide a more joined-up support to communities. It helps us spot things earlier, working proactively through Neighbourhood Networks and Multi-Agency Teams.
